MRCOG Part 2 EMQ Guide: How to Master Extended Matching Questions

Extended Matching Questions (EMQs) make up 60% of the MRCOG Part 2 marks. Yet many candidates spend most of their preparation time on SBAs because they feel more familiar. If you want to pass Part 2, you need a deliberate EMQ strategy.

EMQ Format

Each EMQ consists of a theme (e.g. "Management of antepartum haemorrhage"), a lead-in statement (e.g. "For each clinical scenario, select the single most appropriate initial management"), and 10-14 options. You then receive 5 clinical scenarios, each mapping to exactly one option. Options may be used once, more than once, or not at all.

Why EMQs Are Harder Than SBAs

  • More options: with 10-14 options instead of 5, random guessing gives you a 7-10% chance instead of 20%
  • Pattern matching required: you need to recognise clinical patterns and match them to management pathways
  • Interconnected knowledge: EMQs test your ability to differentiate between similar conditions, not just recall isolated facts
  • Time pressure: each EMQ theme with 5 scenarios takes longer to process than 5 individual SBAs

EMQ Strategy: The Elimination Method

  1. Read the lead-in first: this tells you what category of answer to look for (diagnosis, investigation, management, complication)
  2. Scan all options: mentally group them into categories before reading scenarios
  3. Read each scenario carefully: identify the key discriminating feature that points to one specific option
  4. Eliminate confidently wrong options: narrow from 12 to 3-4 before making your final choice
  5. Look for the single best match: there is always one option that fits better than the others

High-Yield EMQ Themes

Certain themes appear repeatedly in MRCOG Part 2 EMQs:

  • Management of antepartum haemorrhage
  • Causes of postpartum haemorrhage
  • Management of hypertensive disorders in pregnancy
  • Investigation of subfertility
  • Management of ectopic pregnancy
  • Causes of abnormal uterine bleeding
  • Fetal monitoring interpretation
  • Management of labour complications
  • Gynaecological oncology staging
  • STI diagnosis and management

Practice Tips

The single most effective way to improve your EMQ performance is to practice under timed conditions. Set a timer for 15 minutes per EMQ theme (5 scenarios). This forces you to develop the pattern recognition speed needed for the real exam. After each practice session, review every scenario (including the ones you got right) and understand why each option does or does not match.
